  • Insurance – Verify that the patient is eligible and active; Ensure Insurance Profile is accurate in the computer chart
  • Check to see if any family members are due for hygiene or treatment
  • Hygiene Appointments – check patient history, age, and treatment frequencies – Verify that patient is scheduled correctly for a prophy or periodontal maintenance visit. Such as, Anyone over 14 should be an adult prophy & an FMX
  • X-rays - check if they're current for the situation or if updated x-rays are needed
  • Photos - see that all patients have current intra-oral photos (vital for getting claims paid)
  • Perio Chart: is it up to date and performed at least once a year?
  • SRP (Periodontal services) - check that there are current x-rays and six-point perio charting on file to support claims
  • Treatment Appointments:
    • Make sure assigned providers on the appointment match the provider indicated on the schedule
    • Make sure the appointment describes exactly what the patient is scheduled for and that the procedures are selected for the appointment from the treatment plan. Are there necessary pre-operative radiographs, perio charting, and photos in the chart?
    • If the treatment plan has or needs to be signed by the patient or the guardian
    • Indicate the copayment amount to be collected at the time of visit or indicate if the treatment plan was pre-paid and by whom
    • Consent forms to be signed as needed (digital forms only)
    • Verify Health History and Patient Information updates are necessary. Usually, every six months can also be every 12 months (digital forms only)
    • Covid-19 screening form (digital forms only)
    • We will add a note to the appointment for any procedures requiring a post-op x-ray or photo to remind assistants.
